PROGRAMS AND OUTREACH SERVICES

Help with basic needs for food, housing, prescription drugs

In-home services – Home making, Personal Care & Chore Services. Home care workers provide light housekeeping, meal preparation, laundry, personal care and run errands. The chore service workers cut grass and do snow removal.

Multi-purpose senior centers with daily fitness, education, hot meals, field trips and many other fun activities.

Transportation for medical appointments and personal business.Medical Transportation – patient compliance and follow-up care are supported through door-to-door transportation to medical appointments.

Bilingual Services for Spanish speaking individuals

Vital Support Services – all programs are available to the home-bound. A social worker helps with case management

By applying the Transition to Success™ model of care, our team was able to connect individuals to resources and provide warm-handoffs. 

Following the Transition to Success™ model will ease their transition into retirement and senior living.

In 2013-2014 our Bridging the Gap program enrolled 264 individuals and provided a value of $80,803 in referral resources.

446 food vouchers were supplied to senior clients with a value of $13,380.

251 senior clients were provided assistance with chore services and personal care, allowing them to remain in their own homes in independent living, eliminating any out-of-pocket costs that would have exceeded $58,700. (3,916 hours @ $15/hr.)

In 2013-2014, the Senior Center enrolled 147 clients, providing 3,736 hours of wellness services.

MATRIX SENIOR WELLNESS HISTORY

 

 

Matrix Senior Wellness Services (MSWS), formerly Reuther Older Adult and Wellness Services  was founded in 1953 under the leadership of Walter P. Reuther and the United Auto Workers. MSWS is a social program designed to meet the needs of retirees, not just auto workers, but all the men and women throughout the Detroit area.

MSWS provides a critical social support system directly maintaining and improving the quality of life for the community’s older adults by reducing isolation and promoting independence.  A wide array of programs and services assist them in living independently in their own homes as long as possible.
